use of this software.

This license is a 45-day trial license which entitles you to use PacsOne
Server Premium Edition for evaluation purposes without any fee, for a period
of 45 days after installation. At the end of the 45-day evaluation
period, if you would like to continue using PacsOne Server please purchase
one or more license(s) by sending the following information:

1. Contact email address for the purchased license to be sent to.
2. Number of license(s) you would like to purchase.
3. If you are running Windows, the output of the following shell command:

   C:\>ipconfig /all

4. If you are running Linux or MacOS X, the output of the following shell command:

   $ uname -a;/sbin/ifconfig

5. Whether you need a 32-bit or 64-bit version license.
